Pregnancy referral to midwives
Pregnancy referral to midwives: You do not need an appointment with your GP for a midwives referral.
You can self-refer by completing this form: Western Trust Maternity Self-Referral Form
(please leave this form into reception or alternatively you can e-mail it to reception.Z00657@gp.hscni.net - If you have already forwarded your referral form to your preferred hospital for delivery then contact reception with the date of the first day of your last period. This is so we can call for you when you are due your pertussis vaccine at 15 weeks)
If you are pregnant and not taking pregnacare/folic acid then please contact reception and request a prescription for folic acid (a recent height & weight will also be required)

Sexual Health Referral
Omagh Hospital and Primary Complex currently does not have a Sexual Health (GUM) clinic.
Sexual Health (GUM) Clinic services within the Western Trust are:

This is a walk in & wait clinic - Monday, Wednesday & Friday - Registration at 8.30am
