MEDICATION REVIEW

 BLOOD TEST MONITORING WITH PRESCRIPTIONS

If you are due blood test monitoring due to recently starting new medication or for annual monitoring of the medication you are taking, you will recieve a letter from the practice requesting you get some blood tests taken.

You can make an appointment with the treatment room for these blood tests via: Tel 02882835525

If you do not get the requested blood tests taken, your medication quantity may be decreased or not issued until your blood results come back to the surgery.

This is for safer medication management and to reduce the risk of adverse effects from your medication.

CONSULTANT/HOSPITAL ISSUED MEDICATION

If you are under the care of a specialist hospital Nurse, Doctor or a Consultant they may have started you on long term medication to manage your condition. When taking this repeat medication that was started by a consultant, you may be asked to get regular blood monitoring done before your next prescription is issued.

You can make an appointment with the treatment room for these blood tests via: Tel 02882835525

If you do not make an appointment for bloods, your medication quantity may be decreased or not issued until your blood results come back to the surgery.

This is for safer medication management and to reduce the risk of adverse effects from your medication.

If you are having problems with your issued medication you should contact your specialist Nurse, Doctor or Consultant directly either via their secretary or you may have been provided with a help line number.

Although your prescription is issued by the practice, this is at the request of your specialist - GP's cannot adjust the dosage or directions of specialist medication without advice and guidance from the health care provider who started you on this treatment.
