REQUEST MEDICAL NOTES


If you require medical notes please send a written request detailing what information you require to Teresa via our admin email address:

Please give at least 2 weeks processing time as we recieve a high volume of requests.

SUPPORT LETTER


If you require further information for PIP/Universal Credit please provide your request via letter or email - GP's will not complete letters or forms during appointments.

Once you provide written confirmation of what information is required, please allow up to 2 weeks processing time as we recieve a high volume of these requests - Please take this time frame into consideration when you are submitting your request.

Please provide your name, date of birth and address on your request.

You can leave a written letter into reception via the postbox at our front desk or email Teresa at: admin.Z00657@gp.hscni.net 

DVLA FORM & HEALTH DECLARATIONS


When you submit your form at reception please inform the member of staff which GP normally completes your form for you. Please allow up to 2 weeks for forms to be completed. The practice is extremely busy due to the high demand for GP appointments - for this reason admin requests and forms are only completed at the end of the day. Please take this time frame into consideration when you leave your form into the practice.

For some forms there may be a charge applied for a GP to complete it. Please be aware that you may be asked for payment at reception before you recieve your form.
